Step 1. Identify employment status and related information.  

1.      What is the job title? ____________________________

2.      What department is this position in? ________________________________

3.      Who is this employee’s supervisor? ­­­______________________________

4.      What is the employee’s FLSA (Fair Labor Standard Act) status? (Exempt/Non-Exempt) __________________

5.      What is the employment status? (Regular, Full-time, Part-time, Intern, Temporary) _______________________

7.      Identify the approval supervisor and approval date of job description. _______________________________________

Step 2. Identify the main objective of this position and the main purpose of the existence of such a position within the company. (One paragraph that summarizes the essential functions of the job position.)

2.      What is the main goal and purpose of having such a position? __________________________________________________________________

3.      What is the main responsibility or goal of an employee in this position? __________________________________________________________________

Step 3. Identify the main responsibilities and essential functions (at least 8) of the job position in order of importance.
